Co-pays for Prescription Drugs1

To help curb the rising cost of prescription drugs, insurers use a formulary, which includes the drugs the plan will cover. These drugs have been approved by the plan’s pharmacy and therapeutics committee (P&T), which is comprised of physicians, pharmacists, and other health care professionals. Many factors go into the committee’s decisions on which drugs to cover or limit access to, including clinical data, practice guidelines, efficacy, safety, and manufacturer price.

In addition to the formulary, payers employ a number of cost-sharing strategies and restrictions to help reduce prescription costs. Drugs usually fall under one of the following tiers, which determine their co-pays:

  • Preferred Generic: Lowest co-pay
  • Generic: Next lowest co-pay
  • Preferred Brand: Higher co-pay
  • Non-preferred Brand: Next highest co-pay
  • Specialty: Highest co-pay (usually a percentage)
  • Not Covered
